IMPORTANT INFORMATION 2017 PROGRAM
School Holiday Program will run from Monday 3rd July - Thursday 14th of July 2017
To book your child into our Holiday Program:
1. Download and complete a 2017 enrolment form for each child (NB: annual requirement only)
2. Download and complete a booking form for each child, indicating which days you would like your child to attend once you and your children have read the brochure and activities information for activities running each day during the current program. These documents are all released prior to each program.
3. Form(s) to be submited in person at the centre due to privacy laws
4. Please provide all supporting documents when enrolling your child. This applies to families who have not previously attended any programs or where circumstances have changed.
This includes copies of:
- Birth Certificate
- Immunisation Records
- Child Health Record, if available (for sighting only)
- Custody Orders, where applicable
- Medical Management Plans (Anaphylaxis, Asthma, Allergy or other) where appropriate
Bookings for the Winter School Holiday Program will open Monday 5th June and are due by the 14th June 2017.
Bookings received after booking closure date, 14th June 2017 will incur a $20 administration fee.
There are limited spaces available for the incursions, so please book early to avoid disappointment
7.30am to 6.00pm Monday to Friday during school holidays (No program on major public holidays)
The daily cost is $57.00 per child per day.This can be reduced by claiming CCB and CCR.
Incursions incur an extra cost, this is shown on the booking form.
All payments are to be recieved prior to the commencement of the program.
Child Care Benefit:
To be able to claim Child Care Benefit (CCB) as a reduced day care fee our service will now require:
Full name, CRN and Date of Birth of the claiming parent Full name, CRN and Date of Birth of each child attending our program Please make sure that details supplied to us are the same as registered with Centrelink. Failure to provide this information may result in full fees being charged.
If you are not currently registered with Centrelink and have not tested your eligibility for Child Care Benefit, it is in your best interest to do so:
Apply for your Customer Reference Number (CRN) Supply this number to our service Also advise dates of birth for claiming parent and children Your attendance can be reported if details supplied.
This will make it easier when claiming your Child Care Benefit as a Lump sum Payment at the end of the financial year.
Refunds for school holiday day care will only be accepted upon presentation of a medical certificate, complete with the appropriate child’s name, and must be received no later than 1 working day after the School Holiday program closes.
This form is to be completed for every child once every calendar year. Includes personal information, medical information, emergency contacts etc. Form can be submitted at any time, but must be submitted prior to a booking being made. Due to DHS requirements, the enrolment form must be handed in at our centre so that staff can sight your child's immunisation record, health record and birth certificate - these must be original and a copy kept on file. Can't find your Immunisation Certificate? You can download a copy from the Australian Childhood Immunisation Register.
- After 6:45 Police/child protection called and charges at $25.00 per 15 minutes.
Form is to be completed for every child each holiday program. Form will be available approximately 4 weeks prior to each vacation period. Bookings will open approximately 3 weeks prior to each vacation period.
Please Note: Any bookings received after the closing date will incur a $20.00 processing fee.
Ensure you get your booking in on time to avoid disappointment or additional fees.

If you have a child with additional needs it is very important you contact us at least 6 weeks prior to booking your child in so that we can discuss your child’s individual needs and carer requirements.
Current families of children with additional needs must arrange a booking for their child no later than 4 weeks before the program commencing.
